Five months pregnant and five years into her marriage to mafia boss Milo, Giovanna overhears him instruct surgeons to remove her kidneys for his declared soulmate Bella. The betrayal deepens when her mother Sophia admits Bella is her own secret daughter, making the two women half-sisters. Giovanna also discovers Milo murdered all seven of their previous children and stole her eggs.
Feigning obedience, Giovanna secretly records three damning audio confessions and books a flight timed for Milo's birthday celebration. After Bella moves into the estate, she repeatedly provokes Giovanna while Milo and Sophia take Bella's side in every dispute. When illness strikes Giovanna, she is banished to the attic, where Bella removes her urinary catheter to frame her and Sophia forces her to kneel.
