Elite surgeon Shaun boards a flight carrying a donor kidney destined for a clandestine transplant to save billionaire Derek Wolf. Before departure, Kim, mother of Jessica — fiancée to Derek's grandson Erik — delays the plane by remaining in the lavatory. When Shaun presses for urgency, Jessica publicly humiliates him.
Midair, Kim suffers a heart attack and Shaun resuscitates her, fracturing ribs in the effort. Instead of gratitude, Jessica demands a degrading apology and threatens to destroy the kidney when Shaun refuses. He is forced to reveal Derek's identity, but Jessica dismisses the claim until she smashes the case and sees her fiancé's grandfather's name on it.
